Welcome to the webpage of the Illinois Hospice-Veteran Partnership!

The purpose of the Illinois Hospice-Veteran Partnership is to form a statewide coalition to provide leadership, technical assistance, and recommendations for program development.  HVP goals include:

Strengthening the relationships between community hospice agencies and Department of Veterans Affairs (VA) facilities

  1. Improving veterans' access to hospice and palliative care across all sites and levels of care and
  2. Educating veterans about end-of-life care.
